Submitting a Sample

To get started, please complete the form below. It is very important that you complete all fields as omitted or partial information may delay results. It is critical that you confirm that the barcode ID on the collection tube is entered correctly.  

Once you've completed the form, please be sure to drop the sample in the mail in the postage-paid envelope. We will register your sample with our lab and provide results via your Basepaws Veterinary portal.
